The fix ensures that the reference count for `IShellItem *pSI` is decremented, preventing a memory leak. For `IFileOperation *pfo` the decrement of the reference count is only attempted when `CoCreateInstance` is successful. Additionally, the gotos have been replaced with nested if/else statements.
