Docs: add notes on populating bpy.app.driver_namespace

Address issues raised by #114155.

Also fix sphinx documentation generation using incorrect identifiers
when looking up references from PyStruct types
(was looking for `bpy.app.bpy.app.driver_namespace.py`).
